An endurant iis stent graft system was implanted during the endovascular treatment of a 71mm abdominal aortic aneurysm. It was reported that during the index procedure, evar was performed and, upon completion angiography, the device was found not to have been delivered to the intended location. The proximal edge of the graft was positioned approximately 6 mm distal to the left renal artery, and a type ia endoleak was identified. An aortic extension etcf2828c49e was placed in desired proximal position with no endoleak observed.Per the physician the cause is undetermined. No additional clinical sequelae were reported and the patient is fine.

Film evaluation summary: the reported inaccurate delivery and type ia endoleak cannot be confirmed based on the limited pre-implant images provided; therefore, the cause of these events cannot be determined.Procedural angiograms demonstrating stent graft deployment, as well as the reported endoleak, were not available for in vivo assessment of the stent graft. It is likely that deployment of the stent 6 mm below the left renal artery contributed to the type ia endoleak; although this cannot be confirmed.Review of the returned films did not identify any obvious characteristics, such as significant neck tortuosity, thrombus, or calcification, that may have contributed to the inaccurate positioning event and endoleak.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
