Intuitive surgical, inc.(isi) did receive the single port monopolar curved scissors (mcs) instrument to perform failure analysis (fa).Fa was able to confirm/reproduce the reported complaint. The instrument was found to have a dislodged mcs coupling at the distal. The entire mcs coupling was found to be dislodged and detached and was not returned with the instrument measured approximately 0.248" x 0.098" in size.No damage was found on the drive rod.The grip knob was manually rotated, and the drive rod was able to move up and down.Visual inspection found no damage on the tube adapter, snake wrist disks and snake wrist cables. The instrument housing was removed and no damage observed.Manual articulation was performed and the instrument moved intuitively with full range of motion in all directions. The probable root cause is attributed to damage during use, which may result from an accidental drop of the instrument or inadvertent collisions with other instruments or hard surfaces.
