Additional Manufacturer Narrative
|
|
Photos provided appear to confirm a structural failure having occurred to the frame just above where the caster arms are welded to the rigid frame. There was a dual fracture to each side which allowed the lower portion of the rigid frame which contains the front caster forks and footplate to become separated from the base frame assembly. This reportedly caused the end-user to lose positioning and fall from the device where they reportedly sustained a laceration to the upper portion of their back.It is unknown at this time if medical intervention was required. At time of report, permobil has not been able to evaluate the device, thus unable to reach a determination as to possible root cause. If any new information is received, a follow-up report will be submitted.
