It was reported that the procedure was to treat a heavily tortuous right radial artery.A hi-torque versaturn guide wire and ht bmw universal ii guide wire were introduced and advanced through the tortuous segment. It was noted that the bmw guide wire met resistance with the anatomy during advancement.Upon removal, resistance was noted due to the bmw guide wire winding, and force was applied. The tip separated; therefore, the right femoral artery was successfully accessed, and a 6f sheath was placed.A snare device was advanced through the sheath, and the separated tip was successfully retrieved. No additional information was provided.

The device was not returned for analysis.Production record and corrective and preventive actions (capa) reviews were performed and revealed no indication of a product quality issue.Additionally, a review of the complaint handling database identified no other similar incidents from this lot.Reportedly, during advancement, resistance was noted due to anatomy and the bmw guide wire core separated.Upon removal, resistance was noted and force was applied. It should be noted the hi-torque balance middleweight universal ii guide wire instructions for use (ifu) states: do not: push, auger, withdraw, or torque a guide wire that meets resistance. The force applied during removal of the device seemed to be a reasonable clinical response to the difficulties. As there was no damage noted to the device during the inspection prior to use, the investigation determined the reported difficulties appear to be related to circumstances of the procedure as it is likely that resistance was met with the heavily tortuous anatomy resulting in the reported difficult to advance and ultimately resulted in the reported core material separation.During removal of the compromised device interaction with the heavily tortuous anatomy resulted in the reported difficult to remove. As reported, a snare device was advanced through the sheath and successfully retrieved the separated bmw guide wire.Based on the reported information and results of the complaint investigation there is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.H6: medical device problem code 2017 - excessive force.
