The account reported finding a dull blade in the knee arthroscopy pack; the complaint transcript noted dull blades and an update that the item should be no 11 blade.Medline received no sample or photo for the investigation, production records were unable to be reviewed as no lot number was provided, and a review of the finished good pack build found no issues with the build that likely contributed to the reported issue.Without a sample or detailed pictures, medline was unable to confirm the reported defect, and the cause code was "sample required to determine cause." the vendor response stated that dull condition blades are related to the inherent process variability during the grinding process and that initial adjustments and/or stropper units misalignment could be a factor; defect continues to be monitored and the incident was found to be acceptable. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
