Event or Problem Description
It was reported that this patient with end stage renal disease (esrd) on continuous cyclic peritoneal dialysis "[cc(pd)]" utilizing a liberty select cycler and liberty cycler set for renal replacement therapy (rrt) was hospitalized due to peritonitis.Follow-up with the patient¿s pd registered nurse (pdrn) confirmed the patient presented to the emergency room on (b)(6) 2026 with complaints of abdominal pain and cloudy peritoneal effluent fluid.A peritoneal effluent fluid culture and cell count (wbc = >10,000 u/l) were collected, and the patient was formally admitted. The patient was diagnosed with peritonitis and was treated with intraperitoneal (ip) vancomycin 1000 mg every 3-5 days (based on vancomycin trough level), and ceftazidime 1000 mg daily for 21 days. The patient¿s peritoneal effluent culture result returned positive for klebsiella on (b)(6) 2026, and the patient¿s vancomycin was discontinued. The patient was reportedly still experiencing abdominal discomfort on (b)(6) 2026 and the decision was made to remove the patient¿s pd catheter (not a fresenius product) and place a hemodialysis (hd) catheter (not a fresenius product). The patient was transitioned to hd without any reported issues and discharged home in stable condition on (b)(6) 2026.The patient is recovering from the serious adverse events and is scheduled to begin outpatient hd on (b)(6) 2026.The pdrn stated the cause of the serious adverse events is unknown; however, they were not caused by any fresenius product(s) and/or device(s) deficiency or malfunction.

Additional Manufacturer Narrative
The plant investigation is in process.A supplemental mdr will be submitted upon completion of this activity.  clinical review: a temporal relationship exists between ccpd therapy utilizing the liberty select cycler, liberty cycler set and the serious adverse event of peritonitis, characterized by abdominal pain and cloudy peritoneal effluent fluid, which required hospitalization, antibiotic therapy, pd catheter (not a fresenius product) removal, hd catheter (not a fresenius product) placement and transition to hd for rrt.The etiology of the serious adverse events is unknown; therefore, causality could not be firmly established. However, the pdrn reported the serious adverse events were unrelated to any fresenius product(s) and/or device(s) malfunction or deficiency.Esrd patients undergoing pd for rrt (manual or cycler-based) are at high risk for infections of the peritoneum.Klebsiella is considered normal flora of the gi tract and is typically caused by a touch contamination event, exit site infection, or possibly a bowel source.Based on the information available, the liberty select cycler and liberty cycler set can be disassociated from the serious adverse events. There is no allegation or objective evidence indicating a fresenius device(s) and/or product(s) caused or contributed to the serious adverse events.Furthermore, there was no report a fresenius device(s) and/or product(s) failed to meet the users¿ expectations and/or the manufacturers¿ specifications.
