Event or Problem Description
It was reported that the cardiac resynchronization therapy defibrillator (crt-d) may have inappropriately classified ventricular tachycardia (vt) episodes as supra ventricular tachycardia (svt) and delayed therapy due to stability and onset discriminators. It was also reported that there was right ventricular (rv) lead oversensing and undersensing observed which may have resulted in over pacing and delayed episode detection at times.A lead integrity alert (lia) triggered for the rv lead due to high rate non-sustained episodes and elevated sensing integrity counter (sic). The crt-d and rv lead remain in use.No further patient complications have been reported as a result of this event.
