Updated fields: b4, g3, g6, h2, h3, h6 (medical device problem code, type of investigation, investigation findings, component codes and investigation conclusions), h11
corrected fields: d10, d4 serial (revert this field to blank, as it was inadvertently submitted initially) and d8 (revert this field to blank, as it was inadvertently submitted initially), e2, g2.The iab was returned with the membrane completely unfolded and blood on the exterior of the catheter and between the catheter and the sheath. The sheath was not a maquet product. The extender and pressure tubing were also returned.A photo was provided and reviewed.Three kinks were observed on the catheter tubing approximately 37.1cm, 37.3cm and 57.9cm from iab tip.Additionally visual examination did not identify any condensation inside the returned catheter. An underwater leak test of the balloon, catheter, y fitting, extracorporeal, extender and pressure tubing was performed and no leaks were detected. The iab was placed on the cardiosave pump and the iab fully inflated and deflated. The iab was then pumped for two hours which represents one complete autofill cycle. The iab pumped normally and no alarm sounded from the pump. The evaluation determined kinks in the catheter tubing. It is difficult to determine when or how a kink in the catheter occurs.Although we did not repeat the event in the laboratory setting, a kink in the catheter can cause inflation difficulty or an alarm. The failure mode is addressed in the risk file and is operating within its risk profile. The ifu addresses the reported failure. The investigation does not indicate that the device was inadvertently released as non-conforming or an adulterated product or was a counterfeit. The complaint history review did not identify an adverse trend.Based on the rational provided above, no escalation to the capa process is required.
