During a lead revision procedure of an os ipg, the new lead couldn't be fixed in the header of the ipg despite screwing a few times. The physician opted to explant and replace the ipg.The explanted ipg was decontaminated at an approved decontamination facility and returned to impulse dynamics usa for evaluation.Damage was observed at the top of the ipg on both sides; however, all 6 set screws could be fully torqued and then retracted without issue. The defect reported by the physician could not be replicated or confirmed. The root cause of this event is likely due to improper lead positioning during implant.
