The account reported a "breather pouch - open/incomplete seal." a photo and a physical sample were received for one pack; the tyvek portion was observed completely open, no additional tears or pops were noted, finished good production records and finished good pack build were reviewed, and pack stock check found no issues while component stock check found no inventory. The reported open seal was confirmed by the sample evaluation. It is likely that the pack was mishandled and opened at some point after assembly, and with the information provided medline was not able to confirm at what point the damage occurred; a risk assessment was completed and the incident was found to be acceptable. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
