Event or Problem Description
It was reported that a patient underwent an atrioventricular nodal reentrant tachycardia (avnrt) ablation procedure with a thermocool smarttouch sf catheter which had an irrigation issue during use on the patient. It was initially reported by the customer that the new thermocool smarttouch® sf catheter was not irrigating at all and no fluid was coming out of the tip of the thermocool smarttouch® sf catheter. The caller reported that the lab staff attempted to aspirate the thermocool smarttouch® sf catheter, without resolution. The thermocool smarttouch® sf catheter was replaced and the issue was resolved. The procedure continued with no further issues. There was no patient consequence. The customer's reported occlusion was not considered to be mdr reportable since the potential risk that it could cause or contribute to a serious injury or death to the operator or patient is remote. On 29-jul-2026, additional information was received indicating the catheter was used on the patient and suspected to have the irrigation issue. There was no errors noted from the ngen pump and when ablating, there was a high temperature error. It was then noticed that the tubing had disconnected from the catheter. The catheter was then removed from the body and flushing was attempted unsuccessfully. The correct catheter settings were set on the generator.Based on the information indicating the catheter was used on the patient, the event was reassessed as an mdr reportable malfunction.
