the product was returned for evaluation.Visual inspection and functional testing were conducted on the returned device.Visual analysis of the returned sample determined that the cdh29b device arrived with no apparent damage without stapler retainer.Further analysis on the device shows one staple was missing from the pockets and the breakaway washer was uncut in the anvil. No functional test was performed in order to preserve the evidence. In addition, a tyvek returned along with the device. No conclusion could be reached on what cause the staple to fell out.It should be noted that all devices are inspected 100% for staple presence by an automated vision system and are visually inspected 100% as a final check. In addition, at finished goods the devices are visually inspected based on a sample.A manufacturing record evaluation was performed for the finished device batch number 844d36, and no non-conformances were identified.Additional information was requested, and the following was obtained:
was there a patient consequence? if yes, how was the issue addressed?
could you please clarify if there were any change in the post-operative care of the patient as a result of the event?
no patient consequences are to be deplored on this case, the stapler was changed before use, however causing an overconsumption and a waste of time for the team.
