According to the reporter, intraoperatively and prior to patient use, the adapter broke.A replacement adapter was obtained, and the issue was resolved without incident.Subsequently, an attempt was made to load a 60 mm reload; however, the reload could not be loaded, possibly due to damage to the adapter's center rod.Additionally, a radial reload was used for the first firing.Following firing, the radial reload was difficult to remove from the device.Although initial removal was challenging, the reload was eventually removed. There was no patient injury.
