Additional Manufacturer Narrative
|
|
The device was returned for inspection. The root cause could not be identified.Based on the results of the investigation, the insulation beak failure is a mechanical component problem, but the cause of the problem could not be determined. It is likely that the joint between the beak and the insertion tube has come loose for some reason.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
