This follow-up report is being submitted to relay additional information.\
the following sections were updated: g3; h2; h3; h6; h10\
\
visual examination of the provided pictures identified the explanted stem, cone body, head, shell, liner and screw.Devices were covered in bio-debris.No further evaluation could be made with the provided picture.\
\
review of the device history record(s) identified no deviations or anomalies during manufacturing related to the reported event.\
\
radiographs were provided and reviewed by a health care professional.Review of the available records identified the following: periprosthetic radiolucent lines at the acetabular cup metal-bone interface are consistent with loosening.Bony sclerosis is present at the acetabular roof along with bone spurring at the superolateral acetabular roof. The greater trochanter and lesser trochanter are in closer proximity to the bony pelvis than is typical.A small bone island (likely due to heterotopic ossification) resides adjacent to the lesser trochanter.\
\
a definitive root cause cannot be determined.\
\
if any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor trends.
