Event Description
It was reported that the procedure was performed to treat a lesion in the right tibial artery.When an attempt was made to load the 2.0 x 200 mm armada balloon catheter onto the guide wire, the device would not load onto the wire.It was found that the balloon catheter was cracked where the two hubs meet.An un-specified balloon catheter was used to continue the procedure.There were no adverse patient effects and no clinically significant delay in the procedure.No additional information was provided.

(b)(4).It is indicated that the device is not returning for evaluation; therefore, a failure analysis of the complaint device could not be completed.Based on the reported information, there is no indication of a product quality issue with respect to manufacture, design, or labeling.The investigation determined that a definitive cause for the reported cracked hub and difficulty loading the wire could not be determined.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history revealed no other incidents.Based on the information reviewed, there is no indication of a product quality issue with respect to manufacture, design or labeling.
