It was reported the device did not seal.A left atrial appendage (laa) closure procedure was performed, and a 27mm watchman flx closure device was implanted on (b)(6) 2021.The patient was subsequently discharged. On (b)(6) 2026, the patient presented for a transesophageal echocardiogram (tee) as part of a planned valve repair procedure.During the examination, the tee physician informed the implanting physician of the presence of the closure device and identified a visible leak around the device fabric cap.The closure device appeared to be in a position relatively unchanged from the time of the original implant.Imaging demonstrated a large shoulder (between 1/3 to 2/3) on the device in high-angle views. In addition, a 2mm leak was observed beneath the fabric cap visualized most on the 90-degree tee view. The implanting physician is recommending surgical removal of watchman closure device due to a combination of the leak, device position, and lack of device endothelialization if patient is a candidate for surgical valve repair.
