Event or Problem Description
A nurse reported that under a vacuum pressure of six hundred fifty millimeters of mercury, the plunger could not retract properly, preventing normal silicone oil removal during right eye silicone oil removal procedure.The surgery was completed on the same day after replacing the product with a new one.There was no patient impact.
