D2a: common device name: percutaneous cardiac ablation catheter for treatment of atrial fibrillation with irreversible electroporation; reported here as the common device name exceeded the character limit for designated field.Investigation summary:
with all the available information, boston scientific is unable to confirm cause of the reported clinical observation of guidewire stuck and tissue damage. The device has not been received for analysis; therefore, a technical analysis of the complaint device could not be completed. The media inspection conducted via photo was inconclusive. The distal splines appear intact, with no obvious damage.A guidewire is visibly retained at the distal catheter tip, consistent with the reported guidewire stuck allegation.Material is visible near the distal tip; however, the images do not provide sufficient objective evidence to confirm the reported guidewire stuck or tissue damage allegations.Device history record review:
the manufacturing batch record review confirmed that the device met all material, assembly, and performance specifications.Risk review:
a risk review performed for the farawave device confirmed that the reported adverse event guidewire stuck and tissue damage are known events defined in the product risk management documentation. These event types has been accounted for during product risk analysis to support acceptable risk benefit for the product.Labeling review:
based on the information provided, there is no evidence that the device was used in a manner inconsistent with the labelled indications/instructions for use.Investigation conclusion:
the cause not established cause code was selected based on the information provided within the event description. It is unclear how the farawave may have contributed to the reported event. The device was not received for analysis and the provided media did not provide sufficient objective evidence to confirm the tissue damage or establish a device-related contribution. The media inspection conducted via photo was inconclusive. The distal splines appear intact, with no obvious damage.A guidewire is visibly retained at the distal catheter tip, consistent with the reported guidewire stuck allegation.Material is visible near the distal tip; however, the images do not provide sufficient objective evidence to confirm the reported guidewire stuck or tissue damage allegations.Based on the available information, a definitive cause for the event could not be established.
