Investigation results:
device analysis findings:
the device was returned for analysis.Device/media analysis:
the device was returned for analysis.During its analysis it was observed that the distal end was kinked. Therefore, the reported issue of guidewire kinked was confirmed. To conclude, during product analysis, it was also found that the spring tip was detached and did not return for analysis. This failure did not contribute to the reported issue.Device history record (dhr) review:
it was confirmed that this device met manufacturing specification prior to distribution and there no manufacturing deviations which could have contributed to the reported event.Investigation conclusion:
this investigation has been assigned an investigation conclusion code of cause not established. This conclusion is supported by the following objective evidence: regarding the reported issue of guidewire kinked and the additional failure.Analysis of the returned device concluded that the distal end was kinked, and the spring tip was detached and did not return for analysis.Based on the information provided and analysis performed, cause not established is selected as the cause code for the reported issue and additional failure since it remains unknown what factors or conditions were present at the field that contributed to the observed damages.Furthermore, batch record review concluded that no manufacturing deviations were identified during the manufacturing process.
