Batch review performed on 27 august 2026 stem: amistem h 01.18.138 amistem-h std.Size 8 (k093944) lot 141473: (b)(4)items manufactured and released on 10-jul-2014.Expiration date: 31-may-2019.No anomalies found related to the problem. To date, (b)(4)items of the same lot have been sold with no similar reported event during the period of review.Clinical evaluation approximately 11 years and 8 months after primary cementless tha, the patient presented with pain associated with aseptic loosening of the femoral stem, with no reported traumatic event. The available radiograph shows marked proximal femoral bone rarefaction, particularly in the trochanteric region, together with an apparent distal fixation pattern (“potting”).This configuration is compatible with progressive proximal stress shielding and loss of proximal bone support, while the stem remained preferentially engaged distally.Such progressive bone remodeling may contribute to altered load transfer and eventual loss of stable fixation over a prolonged period.Aseptic stem loosening is a well-documented complication of cementless tha and is generally multifactorial.Based on the available radiographic and clinical information, the exact cause of the loosening cannot be determined with certainty. No device was available for direct investigation, and there are no elements suggesting a defective or malfunctioning device.Root cause:aseptic loosening is a possible adverse event described in the literature following primary joint arthroplasty, and its causes are often unknown or multifactorial. There is no indication that any potential issue with the device may have caused or contributed to the event, and the investigation does not indicate any potentially related manufacturing issue or systemic deficiency.
