Event or Problem Description
The customer reported inadvertent button activation and intermittent standby activation during a ureteroscopy procedure involving an olympus videoprocessor.There was no patient injury reported.
Additional Manufacturer Narrative
The device was not returned to olympus for inspection, however, technical assistance center provided remote troubleshooting, and the reported failure was not confirmed.Based on the results of the investigation, a root cause could not be identified.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Date of event is unknown.Additional information will be provided if available.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
