The customer reported an error b30 during inspection for use involving an olympus flex deflectable videoscope.There was no patient involvement.
Additional Manufacturer Narrative
The device was not returned to olympus for inspection, however, technical assistance center (tac) provided remote troubleshooting, and the reported failure was confirmed.Based on the results of the investigation, a root cause could not be identified.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
