Event or Problem Description
According to the reporter, intra-operatively of a laparoscopic procedure, the handle of two clip appliers could be squeezed but the clips would not emerge from the device and when clips did emerge, they failed to clip the artery.Another clip applier from anotherlot number was used to resolve the issue.There was no patient injury.
