Event or Problem Description
It was reported that "nurses are complaining daily because they cannot get the backing to release from the sure site dressing." "i guess sometimes they end up using only tape since the dressing isn¿t working.".
Additional Manufacturer Narrative
The customer reported that nurses cannot get the backing to release from the sure site dressing and sometimes end up using only tape since the dressing isn¿t working.A sample was received, and medline received 5 ea 57404 dressings without the rest of the components in the kits; all 5 ea of the dressings had the paper frame separated from the dressing itself so they were no longer able to be placed or used.Per the investigation and sample received, medline was able to confirm the reported issue.The issue derives from the raw materials process when the force between frame liner and the film is too weak, and medline is working with the component manufacturing facility for further evaluation and corrective actions.There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again.In an abundance of caution, this is a reportable event.If additional information becomes available this report will be reopened and reevaluated.This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
