It was reported that "item never worked" and "they can turn it on but can't make it go up and down.".
Additional Manufacturer Narrative
The customer stated that they can turn it on but can't make it go up and down, and the complaint transcript states the item never worked.No sample or photo of the defective product has been received; medline tried to reach out to the customer to troubleshoot or to obtain additional information regarding this issue, reviewed pcn history, and reviewed quality control inspections records.Investigation results: not confirmed, and the issue cannot be confirmed at this time.Sample required to determine cause; no product changes were identified that could have contributed to the alleged product failure, no corrective/preventive action was taken, and the division will monitor through trending.There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again.In an abundance of caution, this is a reportable event.If additional information becomes available this report will be reopened and reevaluated.This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
