Two nexiva units from lot 6035629 were returned for investigation.Visual inspection identified deformation of the pink y-adapter on both returned units. One returned unit also exhibited a crack extending from the threaded end of the adapter.Blood residue observed within the returned assemblies indicated that the devices had been used. The observed damage was consistent with the reported leakage during use and with a known manufacturing-related failure mode associated with handling of the adapter during assembly.Manufacturing personnel were notified of this complaint.A review of the device risk management documentation was performed and indicated that the potential risk associated with the reported event had been assessed appropriately.
