It was reported that the septum could not be crossed. It was reported that during a pulse field ablation (pfa) procedure for the treatment of paroxysmal atrial fibrillation, a versacross access solution rf wire was used to obtain transseptal access for pulmonary vein isolation (pvi).Following two unsuccessful attempts to cross the septum using rf energy ("buzz"), the physician increased the rf application duration, repositioned the wire, and confirmed optimal tenting of the septum.Despite three additional attempts, the wire was unable to cross the septum. The device was removed and replaced with another versacross device. The procedure was completed successfully using an alternate versacross connect device. No patient complications were reported, and the patient remained stable throughout the procedure.
