Event or Problem Description
It was reported that the patient presented for an implant procedure.During the procedure, the delivery catheter inadvertently contacted the heart causing ventricular tachycardia (vt).Anti-tachycardia pacing was performed to convert, however, vt developed again and cardioversion was performed to terminate it.After the cardioversion, the device could no longer be successfully interrogated.The device was not used, and a new one was implanted successfully after removing electromagnetic interfering equipment.There were no patient consequences.
