Additional Manufacturer Narrative
According to the customer, the sample port "disconnected" when the clinician was "attempting to draw a sample with a syringe"; after the incident occurred the port was cleaned and the catheter was left in place, with no serious injury, medical intervention, or follow up care reported.Medline quality received photographs capturing the issue and visual assessment revealed that the valve was fully disconnected from the sample port housing, with the luer lock syringe still connected to the valve. The reported issue of detached sample ports was verified by visual assessment of the provided photographs, thus confirming the reported complaint. The root cause has been attributed to rare welding errors during the assembly process, and the manufacturing facility has implemented improvements to the process to prevent reoccurrence of sample port detachment. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
