It was reported that during an arthroscopic rotator cuff repair surgery the surgeon drove the second-row anchor into the shoulder. The guide wire entered the shoulder, but the anchor remained stuck on the guide wire and did not seat.Unable to place this anchor, a second anchor was taken from the same batch; the surgeon succeeded in seating the anchor, but observed a resistance between the guide wire and the anchor.Per complaint reporter there was no harm for patient, operator or third party. The surgery was finished successfully with a new device with the same part number. It was not necessary to switch the surgical technique or do a second surgery.
