Event or Problem Description
The customer returned the colonovideoscope for a separate issue.During the initial evaluation of the device, it was observed that universal cord, connecting tube, distal sheath, air/water cylinder, scope connector cover unit and scope connector case unit had foreign objects.There was no patient involvement.
Additional Manufacturer Narrative
The device was returned to olympus for inspection.Based on the results of the final investigation, a root cause could not be identified.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
