Additional Manufacturer Narrative
According to the customer on (b)(6) 2023 when placing the cap on a needle, the needle bent and "poked her finger through the cap"; due to this, the provider required lab work be done, and no additional medical intervention was reported and no serious injury was reported related to the incident.Medline received one standard hypodermic needle in used condition, and upon a visual inspection of the sample, did see the needle sticking out through the needle cap.The complaint was confirmed, and the issue shown was caused due to the recapping of a dirty needle.Material trending via sap showed no similar complaints during the last 12 months, and the division has been informed and will continue to monitor trends. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
