The customer reported that the rubber of the distal needle is not moving back into the correct place after the end user fills a vacutainer tube with blood, causing the needle to remain exposed and leak blood onto the next vacutainer that the end user is trying to fill; there was no reported death or serious injury associated with this event.Medline received a photograph under the complaint of "rubber not moving to correct place," informed the manufacturer due to not receiving samples, and the manufacturer conducted first appearance testing and rubber cap rebound elasticity testing on retained samples from the same lot.The photograph does not have a clear image of the rubber piece itself but does show blood leaking through while the vacutainer is connected to the blood collection set; retained sample testing passed, with all samples free of damage and defect and the rubber cap successfully rebounding to its initial position. The cause was documented as "comp./part defective, broke" and "dc:supplier error asia (medline brand)," and the review of the last three trend reports identified no trends for the specified material number and corresponding defect type. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
