Product complaint # (b)(4) investigation summary according to the information provided, "it was reported that 25mm drill bit bent while drilling for acetabular screw. No negative outcome and no surgical delay. This is all available information. This is from athey cup specials instruments.Needs replaced".The quickset 1pc flex drill bit 25 was returned to depuy synthes for evaluation, and an investigation was conducted in accordance with depuy synthes procedures.Visual inspection of the returned device found the distal fluted tip deformed. No other defects were observed. The observed condition of the device was consistent with a random component failure that may have been caused by exposure to unintended forces, such as, multiple uses under extreme eccentric loading resulting in premature bending or failure of the drill bit.Properly handling and¿attention to the approved use of the device diminishes the risk of failure. The reported complaint was confirmed based on the evaluation results. The investigation traced the potential cause to unintended use error. It has been determined that no corrective or preventive actions are proposed. There is no indication that a design or manufacturing issue caused the reported condition. As part of depuy synthes quality processes, all devices are manufactured, inspected, and released in accordance with approved specifications.Additional monitoring for potential safety signals is conducted through complaint trend analysis as part of post-market safety surveillance activities.Device history review the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.Corrected: h3.
