According to the customer, when using the incisor blade for a ¿knee scope¿, it broke¿ during the case; the customer reported there was no impact to the patient and no medical intervention or serious injury related to the reported incident.During the investigation, the received device was inspected for nonconformities per internal procedure, and it was observed that the device shaft had completely separated from the hub and cracking was observed on the outer hub where the outer shaft is secured to the device. The reported issue was confirmed. The exact root cause of the damaged device could not be determined; however, a potential root cause of the damaged device/hub was due to side-load being applied to the device during use. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
