Additional Manufacturer Narrative
The customer reported that the dressing included in the kit was used in or and was not pulling apart properly, and that the adhesive plastic is not separating. No sample or photo was received for the investigation; the production record was unable to be reviewed due to no lot number being provided, the finished good pack build was reviewed, and there were no issues with the build that likely contributed to the reported issue. The reported issue could not be confirmed without a sample; due to other reports received, the likely root cause is a vendor issue relating to their manufacturing process.Medline is working with the component manufacturing facility for further evaluation and corrective actions, and the division will monitor for trending purposes. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
