The customer reported that this part of the saline lock has come apart quite frequently in the last month and came apart while hooked up to the patient in the recovery room; the line was sanitized with alcohol and hooked back up to patient, no serious injury, medical intervention, or follow-up care was reported, the patient is fine, and the procedure/medication administration was completed. The customer provided a sample for evaluation, but the sample was either lost or misplaced; the division pulled 3ea of the item for testing, attached the female luer to the male luer of an iv admin set, attached the whole setup to an iv bag, and flushed fluid through each sample for around 30 minutes. For all the samples, the needless connector did not come off or become loose, and based on the complaint details, sample evaluation and investigation, the issue cannot be confirmed. The vendor responded that, by design, the set is designed to allow for the valve to be removed, as it is non-bonded, and medline concluded the item remained attached to the iv admin set for the entirety of testing and recommended use with an iv admin set that can screw onto the female end of the device to avoid possible instances of separation. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
