Event or Problem Description
It was reported that "cant run, 1ea scuffed/cracked, 4ea missing elbow" and "we are missing 4 of the elbows for the lids.".
Additional Manufacturer Narrative
The customer reported "cant run, 1ea scuffed/cracked, 4ea missing elbow," "cannot use," and "missing elbow"; per customer, one of the 2000 cc canisters was "pretty scuffed" and they were "missing 4 of the elbows for the lids." visual inspection was conducted with the provided pictures, and it was found that the item is scuffed.At this time, the reported concern was confirmed, as the item was scuffed.The suspected root cause is undetermined at this time, and the division will continue to monitor the issue for trending.There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again.In an abundance of caution, this is a reportable event.If additional information becomes available this report will be reopened and reevaluated.This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
