The account reported finding stopcocks pulling in air, and the customer reported contrast leaking through the back of the turn knob.A photo and a sample were received for investigation; a video confirmed that the stopcock during use was leaking fluids onto the surface, and closer inspection found the middle compartment of the stopcock has a crack in its surface, likely causing the leakage. The reported problem was confirmed. The item is received from the vendor and is not altered during the kit assembly process; the nature of the damage indicates a potential molding defect, and the component manufacturing facility identified that the leaking is caused by a mold deviation which leads to uneven wall thickness of the item. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
