The reported failure was pump malfunctioning; the complaint stated the pump was malfunctioning and consistently alarms, with no loss of air in the mattress.The investigation documented that the unit was out of warranty, the complaint was reopened due having the lot number for further investigation, and the mattress was manufactured april 2024 and has been in use with the customer since (b)(6) 2025, which would not be an out of box quality defect.The reported problem was unconfirmed, with sample required to determine cause; no root cause was identified.The item is discontinued and the div is no longer working with this vendor for this product code.There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again.In an abundance of caution, this is a reportable event.If additional information becomes available this report will be reopened and reevaluated.This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
