Additional Manufacturer Narrative
Based on the final investigation of the device the initial evaluation finding were confirmed.A root cause for the reportable malfunction, scope communication error (b30), identified during the device evaluation was traced to corrosion of the plug unit. The most likely cause of the noisy image was damage to the charge-coupled device (ccd) unit. The most likely cause of the no illumination condition was breakage of the light guide bundle.Additionally, the corrosion on the connecting tube was traced to component failure.Olympus will continue to monitor the field performance of this device.Should any additional relevant information become available, a supplemental report will be submitted.
