The customer returned the gastrointestinal videoscope for a separate issue.During the initial evaluation of the returned device it was observed white residue inside the air/water channel, white residue inside the air/water cylinder and noisy in the image.No patient involvement.

The device was returned to olympus for inspection and during the device evaluation, white residue inside the air/water channel, white residue inside the air/water cylinder and noisy in the image was found.A root cause could not be identified.Based on the results of the investigation, olympus confirmed foreign material came out of the device, but the type of the material or root cause cannot be identified.A definitive root cause cannot be determined.A root cause could not be identified.Based on the results of the investigation, it is likely the following led to the malfunction: component failure.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
