Event or Problem Description
The report states, "we ordered 16 boxes of these at the end of july.We use these mainly for the bup.There were at least 5 in one day that came back negative for this drug.We had to send these out to our lab for confirmation, which did come back positive for the drug.This occurred several days.Drug cups are no longer used due to false negative results.".

The report states, "we ordered 16 boxes of these at the end of july.We use these mainly for the bup.There were at least 5 in one day that came back negative for this drug.We had to send these out to our lab for confirmation, which did come back positive for the drug.This occurred several days.Drug cups are no longer used due to false negative results." customer reported, "negative urine sample was to be sent to diagnostics for confirmation testing to ensure patient compliance with controlled substance use as policy." there were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again.In an abundance of caution, this medwatch is being filed.If any further relevant information is identified or obtained, a supplemental medwatch will be submitted.
