Additional Manufacturer Narrative
The account reported finding fluid is not pushing through tubing.A sample was received for the investigation; the customer provided 2ea of dyndtc5077 and a needless connector, and sample evaluation observed no flow of fluid through the set-up until heavy pressure was applied to the plunger of the syringe.Based on the complaint details, sample evaluation, and the investigation, the complaint is confirmed because the returned customer samples could not be flushed under gravity-pressure.A root cause for the issue cannot be determined; the manufacturer provided their investigation and did not find any discrepancies within their records that could have contributed to the reported issue, and the division will continue to monitor the issue for trending. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
