The customer reported multiple incidents of the needle "loosening prior to administering" when attaching the needle to "pre-filled influenza vaccine" syringes, and reported that this issue led to the needle "popping off" and "propelling through the air" after administering the vaccination. No samples or photos were provided; investigation included review of the lot/serial number and recent complaint trends/scar activity for the past 12 months, and per vendor, through sample investigation and dhr review, no abnormalities were found and the product quality meets the requirements of product specifications. The complaint was not confirmed.Per vendor, the safety injection needles produced by caina have no quality issues, and the vendor inferred that the needle detachment was caused by the customer not tightening the safety injection needle and syringe during use; based on review, medline was unable to confirm the complaint issue or that the product failed to meet specifications, and there was no reported death or serious injury associated with this event. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
