It was reported that the screw that holds the bed from going down when pressing the release button is loose and won't screw in properly, and it will not hold the rail in the position that they choose anymore; no injuries were reported.Medline received 1 each of xtra tall rails in used condition and checked this, determining it is not a screw but a locking pin that fell out; the customer used black tape to hold it in place, and the product was inspected per pic-00374.The customer complaint was confirmed, and the result of the pic testing was functional defect. The locking pin fell out due to a missing c-clip, which is needed for the locking mechanism to work as designed; the c-clip was not returned, and the cause was recorded as cannot determine cause w/received sample. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
