Event or Problem Description
During an atrial fibrillation procedure, air was introduced from the swartz sl0 sheath.It was noted that a syringe was attached to the side port to aspirate when the air entered the sheath.The sheath was exchanged, and the procedure was completed with no adverse patient consequences.
