Additional Manufacturer Narrative
The customer contacted medline regarding their bed and stated "height actuator broke," and the complaint also notes a broken actuator hi-lo foot section. The customer provided photos of the hi-lo actuator, which appears in used condition; a physical sample was not provided, so only a visual review of the defect was conducted and a functional or dimensional inspection could not be executed via a photo.Investigation results were confirmed, and after reviewing the photos it was discovered that the hi-lo actuator metal bracket that connects to the bed frame is fractured off.Based on the proof of purchase provided, the unit is out of warranty for electrical components, therefore this issue could be due to end of life expectancy or normal wear and tear; no product changes were identified that could've contributed to this product failure and additional recommendations were none. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
