This is a combination product.According to the facility, during preparation for use, the 9 fr introducer was being prepped when an attempt was made to insert the blue obturator/dilator; the blue obturator/dilator would only advance approximately two inches into the introducer, and several attempts were made to insert the obturator/dilator and were unsuccessful. The issue was identified during catheter preparation, prior to use on the patient; no patient injury was reported, and no medical intervention, serious injury, or follow-up care has been reported.Sample evaluation found that the dilator was unable to advance through the catheter; the catheter was found to have an undersized inner diameter in all tested samples, and two of four dilator samples tested were found to be oversized.Based on the complaint details, sample evaluation, and investigation, the complaint is confirmed; the reported condition is confirmed to be component manufacturing-related based on the nature of the defects observed, however the samples received were insufficient to determine a specific root cause. There were no reports of death, serious injury, medical intervention, or follow up care.It has been determined that the reported event could cause or contribute to serious injury if it were to occur again. In an abundance of caution, this is a reportable event. If additional information becomes available this report will be reopened and reevaluated. This medical device report (mdr) is being submitted as part of retrospective review activities, which include review of historical data in accordance with regulations at 21 cfr part 803.The information included in this mdr reflects the information reasonably known to the manufacturer at the time of this retrospective review and submission.
